Insurance Plans
We want all of our patients to feel welcomed, safe, and that they've received exceptional care. We require an initial visit to address any current medical issues, review current medications, and review past medical history. This is not your annual physical (what we call a “wellness visit”) in which the focus is more on prevention than on ongoing problems. At your first visit, we will set the timing for when you should return for the physical/wellness visit. Occasionally, patients have no significant medical issues so the first visit can be turned into the “physical'“, but do not plan on this and it is at the discretion of our providers. We have this policy to ensure a safe and thorough transition into our practice.
We are In-Network and accepting new patients with most insurance plans. It is your responsibility to contact your insurance to verify participation with our supervising physician Dr. Chad Costley. If you have a HMO Plan that requires you to select a PCP it is your responsibility to call and have the PCP updated to Dr. Chad Costley. This must be effective on the date of your visit. We do not accept Medicaid at this time.
